Our Palliative Care Team works in multidisciplinary fashion to provide the best supportive care to prevent and relieve sufferings for patients and their families which consists of two psychiatrists, three medical oncologists, one anesthesiologist, two radiation oncologists, five palliative care nurses, one pharmacist, four social workers and three physical therapists. After receiving the consultation from the primary oncologists, the problems, goals, and solutions are discussed for each patient in weekly conference.

Once the palliative nurses as case manager observe any problems or severe symptoms, they will coordinate with the primary oncologist to jointly come up with the best alternative care and treatment. If patients still have complicated issues, there will be a discussion in weekly rounds in coordination with other related physicians and supporting staffs holistically.

The team also does home visits after discharge for continuation of care with the team networks in health centers. For referral patients who receive a treatment in OPD and IPD, making a follow-up call with re-assessment is done regularly until the end of life. Our team has good relationships with patients and their families. For some cases where patients don't live in Bangkok and peripheral areas that we cannot visit by ourselves, communicating with the healthcare team of the Ministry of Public Health for home visits would be connected promptly.
