This new Young Oncologist project challenges your capability of diagnosis based on a striking and clinically important image.

You are presented with an image accompanied by a question giving you the opportunity to test if you can correctly identify the right diagnosis. You will then discover the diagnosis that was actually made together with a brief background to the medical history of the case.

ESMO members can rate the article and leave comments to raise questions concerning the diagnosis or share their own experience of a similar case.

A 40 years old female treated with docetaxel, cisplatin and 5FU (TPF) based chemotherapy for locally advanced oral cavity cancer presented abnormal behaviour, dysphasia and dysarthria on the 2nd day of 4th cycle of chemotherapy. MRI brain was done.

61 years old patient with st.IV ovarian carcinoma underwent neoadjuvant chemotherapy followed by Interval Debulking Surgery (IDS).2 weeks post IDS, patient appeared with dyspnea. X-rays revealed pleural effusion, chest tube was inserted on the right side. One-hour post chest tube insertion, dyspnea worsened.

A 52-year old man tested positive on screening faecal blood test. Endoscopic evaluation revealed multiple polyps throughout the stomach and colon, furthermore a large bulging mass in the gastric corpus was found. A preoperative computed tomography was performed.

Female patient, 59 years old, with relapsed Non-Hodgkin Lymphoma (NHL). She developed tumour lysis with acute kidney injury after chemotherapy with ifosfamide, carboplatin and etoposide (ICE). In addition, she suffered severe pancytopenia. However, despite maximum supportive care, including granulocyte-colony stimulating factor (G-CSF) as well as renal dialysis, her platelet count, haemoglobin and creatinine did not improve. Subsequently, she developed leg lesions.

A 4-year old boy with swelling in the left thigh for 4 months. No history of fever or trauma. X-ray revealed a lobulated, expansile, osteolytic lesion involving shaft and lower end of left femur with periosteal reaction and minimal soft tissue component.

A 59-year-old man with non-productive cough and chest pain of one month of evolution. He was diagnosed with musculoskeletal pain and received analgesic treatment showing mild improvement. In light of the persistence of pain, a chest X-ray was requested.

61 year-old man with diagnosis of acral lentiginous melanoma in the right hand index pT4bN1M0 (Stage IIIB) was treated with surgery. We proposed adjuvant treatment with interferon therapy and medical follow up, but the patient refused both. Two years later, he started with neurological symptoms: diplopia, facial pain, paresthesias and numbness and weakness of the upper and lower face, bilateral sensorineural deafness, along with dysarthria, dysphagia and hoarseness of voice. On the brain MRI we observed the following findings.

This 24 yrs. male had a painless progressive lump on the medial side of left elbow for 1 year. It had recurred after 2 local excisions. The histopathological examination after second excision was ‘poorly differentiated malignant tumour’. Physical exam, complete blood count and peripheral blood smear were normal. Biopsy showed a small round cell tumour. Final pathologic diagnosis was established by IHC. The tumour resolved after chemotherapy.
